WORK STATEMENT: Place of Performance: Polk Street Loading Docks Jesse Brown VA Medical Center 820 S. Damen Ave Chicago IL 60612 Part 1: Project Scope of Work Overview Contractors shall provide all tools, materials, components, labor, and supervision to remove and replace the 2 existing/nonfunctioning lifts. 1 lift is used with the Compactor and 1 lift is used for the Linen Truck and deliveries on the Polk Street Dock. Part 2: Period of Performance (POP) Complete all work including submittals, reviews, and quality control immediately after notice to proceed. Part 3: Detailed Project Scope of Work Contractors shall provide all tools, materials, components, labor, and supervision required to install and perform the following: Task #1 Remove and dispose of (off Station) the existing nonfunctioning lift for the Polk Dock Compactor. Replace with One (1) new Recessed Dock Lift 5,000 lb. capacity. Capacity: 5,000 lbs. Axle Capacity Ends: 2,500 lbs. Axle Capacity Sides: 2.500 lbs. Platform Size: 6 x 8 ft. Lowered Height: 8 in. 5 HP Motor Approx. 12 FPM Speed Push button controls Electrical hook ups of new equipment using existing conduit and wire. Secure the new lift into place, ensure that it is level and secure, install all other necessary hardware, make necessary adjustments, and perform our Standard Quality Assurance & Safety Check to ensure safe and proper operation of the Unit. Contractors shall demonstrate functionality before leaving the station to the POC. 1 year warranty minimum Task #2 Remove and dispose of (off Station) the existing nonfunctioning lift for the Linen Truck on the Polk Dock. Platform Size: 98" x 120" Lowered Height: 10 in. 5 HP Motor Approx. 10 FPM Speed Push button controls 3. Electrical hook ups of new equipment using existing conduit and wire. 4. Secure the new lift into place, ensure that it is level and secure, install all other necessary hardware, make necessary adjustments, and perform our Standard Quality Assurance & Safety Check to ensure safe and proper operation of the Unit. 5. Contractors shall demonstrate functionality before leaving the station to the POC. 6. 1 year warranty minimum Part 4: Applicable Performance Standards Construction Safety and VA Infection Control Policies. 1. All work shall comply with all VA Policies including at minimum the most current: VA Construction Safety and VA Infection Control Policies. 2. The Prime Contractor shall always have a Competent Supervisory Person on site when any worker(s) or sub-contractors are present. 3. All Persons working on any electrical systems rated 50 volts or more shall have a minimum qualification as a State Licensed Master Electrician or Registered Journeyman Electrician. All activities shall follow the safe work practices in compliance with most current 70E Electrical Safety in the Workplace. 4. The Contractor shall provide mitigating activities. a. No trash used packaging or construction spoils shall be stored on site. Remove each day using covered gondolas. b. Broom sweep the work area at the end of each day. 5. During work execution, the Contractor shall take care to protect Government Property Damage resulting from Contractor operations shall be repaired by the Contractors, including painting, refinishing, or replacement (if necessary), at no cost to the Government. Part 5: Hours of Operation 1. The Contractor shall comply with all Jesse Brown VAMC site specific requirements and SOPs which include at minimum: Key Policy, Badging Policy, Rules of Behavior, and Smoking Policy. 2. Normal work hours shall be 7:00am to 4:30pm local time Monday through Friday. a. Normal work hours shall be modified only by written approval of the COR. b. There shall be no work by the Contractor on Federal Holidays and there shall be no work on the weekend when the Federal Holiday is on the respective Monday or Friday. Federal Holidays include: New Year s Day Labor Day Martin Luther King Jr. s Birthday Columbus Day Veteran s Day Memorial Day Juneteenth Day Thanksgiving Day Independence Day Christmas Day Any other day specifically declared by the President of the United States to be a Federal Holiday. When a holiday falls on a Sunday, the following Monday shall be observed as a Federal Holiday by U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a Federal Holiday. c. If normal work hours are modified the Contractor shall check in (in person) with the Administrative Officer of the Day (AOD) which is in Patient Admitting for any work between 4:30 pm through 7:00 am. Provide the name and cell number of the on-site supervisor to the AOD. At the completion of the work, check out with the AOD. Part 6: Check In/Check Out a. Contractor shall present a valid form of Id at the Information desk to Badge In. The information desk is located at the exterior entrance to Bldg. 2, Ogden Circle Drive Main Entrance. b. Contractor shall notify the POC upon arrival by phone 312-569-7574. c. The POC will then escort the Contractor to the work area. Part 7: Documentation Documentation/Invoice shall be submitted to the CO and COR via email no more than 5 business days upon completion. The Invoice shall contain make and model number of the equipment installed, and all work done.
