<p>MetroPlus is seeking vendor(s) to assist in implementing risk related activities which ensure that the member populations under care in the Medicare, Medicaid and Affordable Care Act markets, are represented accurately when it comes to chronic conditions and diagnoses. This Request for Proposals (RFP) seeks a vendor partner(s) that will support MetroPlus and lead efforts for either or all the following services:</p><p>1. Population Analytics</p><p>2. Supplemental Chart Reviews (including retrieval)</p><p>3. In-Home Assessments</p><p>Proposers have the ability to submit proposals for either or all options listed above.</p>
