HHSC is seeking Applications to establish Contract(s) for the provision of Home and Community-Based Services - Adult Mental Health (HCBS-AMH) for adults with a diagnosis of serious mental illness and have a history of at least one of the following additional criteria: Long-term psychiatric hospitalization(s);Frequent arrests and psychiatric crisis; andFrequent emergency department (ED) visits and psychiatric crisis. The HCBS-AMH program serves adults with an array of services matched to each person's needs. The goal is to help the person live and experience successful tenure in their chosen community and support long-term recovery from mental illness.
